Support
Wish Recipient is under the care of
a Palliative Care Service who has partnered with AWSA
Wish Recipient Location
Care location of the Wish Recipient is within the geographical boundaries covered by the Palliative Care Service who has partnered with AWSA
Wish Destination
Location of the Wish site should be within 150km from Wish Recipient’s pick up location
Diagnosis
Wish Recipient is diagnosed with a life-limiting condition and/or a prognosis of less than 6 months
Mobility
Wish Recipient is unable to self-mobilise OR require a minimum of 1 person-assist for mobility/transfers OR spends more than 60% of time in bed OR limited by fatigue
Goals of Care
Wish Recipient has clear and up-to-date Goals Of Care documented/ Care Plan in place (including not-for-resuscitation order)
Consent
Wish Recipient OR their Substitute Decision Maker (appointed legal representative) is able to consent to the Wish
Wish Request
Wish must be low in complexity and risk

Ambulance Wish with Specialist Needs Wishes (including Clinical Escort)
Specialist Needs wishes are for Wish Recipient’s with needs that require a clinical escort.
Specialist Needs Wishes are fulfilled in partnership with your Palliative Care Service Provider, they will provide a clinical escort for the wish, as a continuity of their palliative care service for you.
How to request a wish:
- If you are a patient of Palliative Care Service – Please talk to your service provider to discuss an Ambulance Wish.
- If you are a Palliative Care Service staff member and have discussed a Ambulance Wish with your client, please discuss this with your designated Ambulance Wish SA Liaison (usually your NUM) and organise for the Wish Request Form below to be completed.
Please note: The Specialist Needs Wish Request Form should ONLY be completed by the Palliative Care Service Staff
