Support
Wish Recipient is under the care of
a Palliative Care Service who has partnered with AWSA
Wish Recipient Location
Care location of the Wish Recipient is within the geographical boundaries covered by the Palliative Care Service who has partnered with AWSA
Wish Destination
Location of the Wish site should be within 150km from Wish Recipient’s pick up location
Diagnosis
Wish Recipient is diagnosed with a life-limiting condition and/or a prognosis of less than 6 months
Mobility
Wish Recipient is unable to self-mobilise OR require a minimum of 1 person-assist for mobility/transfers OR spends more than 60% of time in bed OR limited by fatigue
Goals of Care
Wish Recipient has clear and up-to-date Goals Of Care documented/ Care Plan in place (including not-for-resuscitation order)
Consent
Wish Recipient OR their Substitute Decision Maker (appointed legal representative) is able to consent to the Wish
Wish Request
Wish must be low in complexity and risk
Ideas for Wishes
We can either take you to your wish or bring a wish to you. If you have a wish that you would like Ambulance Wish SA to fulfill and meet our eligibility criteria, then talk to your specialist palliative care service.
Wishes are individualised and usually have a special connection/meaning for the wish recipient, some ideas:
- Visit their home/pet
- Visit their child/grandchild/great-grandchild
- Visit the beach/ocean
- Visit the botanic gardens
- Visit a gallery or museum
- Bring your pet to you
- Bring your favourite meal from your favourite restaurant

How to request a Wish
- Check that you meet the wish eligibility criteria
- Identify your wish
- Ask to talk to your Ambulance Wish SA Liaison at your Specialist Palliative Care Service
- Your Palliative Care Team will complete a Wish Request Form and send this to us.
Wish Types Options:
OFFSITE Wish Type
Offsite: Wish fulfilled in another location other than the Palliative Care Provider’s facility
Offsite Wish example: Travel in Wish Ambulance to the beach with family or even to their family home for a visit and catch up with loved ones
ONSITE Wish Type
Onsite: Wish fulfilled onsite in Palliative Care Provider facility
Onsite Wish example: A small party for the Wish Recipients children is held in the hospice or a residential aged care facility OR bagpipes are played by the Wish Recipients bedside.
COLLECTIVE Wish Type
Collective: Multiple Wish Recipients wishes are fulfilled at once. (Note these are usually onsite wishes)
Collective Wish example: Multiple Wish Recipients in hospice/Aged Care Facility receiving a special photoshoot/ videographer and memory books/videos for Mother Day.
Wish Engagement Level Options:
HIGH Level Engagement
- PHOTOGRAPHY: Photographer/Videographer to attend the wish, images provided to the Wish Recipient and Family on a USB and available for program use
- WISH MEMORY BOOK: Wish Memory Book to be created and provided to Wish Recipient & Family and those involved in the Wish
- WISH MEMORY STORY SHARING: Wish story on website and social media sharing wish experience, including wish recipients first name. Images shared as well.
- VIP: This may include sharing the wish story with Media and/or Members of Parliament (MP), who may share or attend the beginning of the Wish with an aim to raise the profile of the AWSA program
High level engagement helps our charity program to share our Wish activities and Wish stories while also promote donations, sponsorships and fundraising activities.

STANDARD Level Engagement
- PHOTOGRAPHY: Photographer to attend the Wish, images provided to the Wish Recipient and Family on a USB and are available for program use
- WISH MEMORY BOOK: Wish Memory Book to be created and provided to Wish Recipient & Family and those involved in the Wish after the Wish
- WISH MEMORY STORY: Wish story on website and social media sharing wish experience, including Wish Recipients first name. Images shared as well.
Standard level engagement helps our charity program to share our Wish activities and Wish Stories

LOW Level Engagement
- PHOTOGRAPHY: Photographer to attend the wish, no images shared other than to the Wish Recipient and family.
- WISH MEMORY BOOK: Wish Memory Book created but only copies for the wish recipient and/or family, no other disseminated. All images will be destroyed once provided to the wish recipient/and or family.
- WISH MEMORY STORY: Wish story on website wish experience only – only include first letter of persons name, what the wish was and with team involved. Nothing identifiable. Nothing on social media.
Low level engagement helps our charity program to share our Wish activities and provides examples of wish types.

- PHOTOGRAPHY: No Images to be taken on the wish
- WISH MEMORY BOOK: no wish memory book created

REQUEST A WISH
Specialist Palliative Care Needs Wishes
Specialist Palliative Care Needs wishes are for Wish Recipient’s with high clinical needs that require a clinical escort.
Specialist Palliative Care Wishes Needs are fulfilled in partnership with your Palliative Care Provider, they will provide a clinical escort for the wish, as a continuity of their palliative care service for you.
How to request a wish:
- If you are a patient of Palliative Care Service – Please talk to your service provider to discuss a Ambulance Wish.
- If you are a Palliative Care Service staff member and have discussed a Ambulance Wish with your client, please discuss this with your designated Ambulance Wish SA Liaison and organise for the Wish Request Form below to be completed.
Please note: The Request an Ambulance Wish for Specialist Needs Form should ONLY be completed by the Palliative Care Service
Generalist Palliative Care Wishes
Generalist Palliative Care Services wishes are for Wish Recipient’s with clinical needs that can be provided by their carer (family member, friend etc).
This needs to be approved by their health care lead (usually their GP) and the GP needs to complete a consent form in addition to the Wish Recipient.
